Referrals

This section is intended for reentry specialists, case managers, social workers, and TDCJ staff coordinating post-release support. If you're assisting an individual transitioning back into the community, please use this form to submit a referral for housing and wraparound services through Second Wind Living.

Know someone who could benefit from transitional housing through Second Wind Living? We welcome referrals from friends, family members, neighbors, and community members—not just professionals. If your referral leads to a successful placement, you’ll receive a $50 check by mail as a thank-you for helping us support someone’s fresh start.

Just complete the referral form with accurate contact details and basic information about the individual. We will take it from there.
